To purchase a plate with the qualifying disabled symbol, just follow the 4 easy steps below:
- Click here to print and then complete the application form.
- Ask your doctor to verify and sign the Disability Statement section on the application.
Send a copy of the application to My Plates by mail, fax or e-mail including your contact details. Once the form has been received, one of our friendly
customer service representatives will call you to assist in the ordering and processing of your new plate.

- Once your plate has been processed and delivered to your local county, you will need to provide the county office with the original copy of the signed 214 form at the time of collection.
