How to Apply: A written request with a copy of the applicant's U.S. Department of Defense Form DD-214 or a copy of the award certificate must be submitted to Vehicle Titles and Registration, 4000 Jackson Avenue, Austin, TX 78732

All printed plate applications must be mailed to the address referenced on your form (either your local County Tax Assessor-Collector office or the TxDMV). The address of the TxDMV and your local Tax office can be found by clicking here.
